Wrestler Information FormEach year parents are asked to fill out an Information Form for wrestlers involved in the Columbus Wrestling Organization. We need to have this information available so we can develop a contact list and we also need the physician information available for practices/tournaments in case of an emergency. NOTE: The information form also contains a place to order your wrestler a free short-sleeve team t-shirt. If the information form is not returned by Friday, January 11th, they will miss the first t-shirt order and will have to wait until the second order arrives. Please fill out and return the Columbus Wrestling Organization – Wrestler Information Form for your wrestler(s). The link below can be used to fill out and submit the form electronically or can be printed and turned in at practice. Please submit the form as soon as possible when you check in at practice. |
